All Clients: please complete this COVID Intake Form within 24hrs of your session.

MVA Insurance Clients: please complete this MVA Insurance Form.
Please wear or bring clothing that allows me to see and work with your physical structure such as underwear, shorts, sports bra, etc. Your comfort is MOST important so please plan accordingly and contact me if you have any questions.
Please refrain from perfumes and fragrances as I and several clients have chemical sensitivities.
COVID Safety
Prime Vital Health has put in place preventative measures to reduce the spread of COVID but social distancing is NOT possible during your bodywork session. Please help ensure the safety of yourself and others by following these guidelines:
Please print and fill out this COVID Intake Form 24hrs prior to your session and bring it with you. You will be asked to date and initial this form at the beginning of every session.
Please arrive wearing a well maintained and fitted safety mask that covers both your mouth and nose; I will meet you wearing mine. You will be asked to remain masked for the entirety of your session and I will do the same.
Please bring proof of COVID vaccination status, and/or proof of natural COVID immunity within the past 3 months, and/or proof of a negative COVID test within the last 24hrs with you to your session.
For the safety of vulnerable people in our community, and with few exceptions, I only see clients who are fully vaccinated, and/or have gained natural immunity through a prior COVID infection, and/or have recently tested negative for COVID.
If any of these guidelines are problematic for you due to health or medical reasons, please contact me to discuss options.
We reserve the right to refuse service to anyone who does not or will not follow these guidelines.

Prime Vital Health Studio is located on the second floor of Taborspace, a community center housed in a beautiful old church building at the intersection of 54th and SE Belmont St., Portland, OR 97215.
60 Minute Rolfing Session: $165
This rate reflects a time-of-service discount for paying at the time of service. The full session rate is the most current rate set for massage therapy and manual therapy by the State of Oregon Worker's Compensation Fee Schedule.
